Terms of Service for Francis Wellness Services LLC

These Terms of Service ("Terms") govern your use of the services provided by Francis Wellness Services, LLC, a mental health and therapy practice. By scheduling an appointment and engaging in therapy with us, you agree to the following Terms:

1. Services Provided

We provide outpatient psychotherapy, mental health assessments, and substance abuse treatment services. We work with individuals seeking support for various mental health concerns, including but not limited to anxiety, depression, trauma, and substance use.

2. Client Responsibilities

As a client, you are responsible for:

  • Attending scheduled therapy sessions or notifying us at least 48 hours in advance if you need to cancel or reschedule.

  • Providing accurate and complete information regarding your health history and personal circumstances.

  • Actively participating in your treatment, including completing any assigned homework or exercises.

3. Payment and Insurance

  • Fees: You are responsible for paying the fees associated with your therapy sessions. The standard fee is [Insert Fee]. Payments can be made through [Insert Payment Methods].

  • Insurance: We do not accept insurance directly; however, we can provide you with superbills that may be reimbursed by your insurance company. You are responsible for checking with your insurer regarding coverage.

4. Confidentiality

5. Limits of Confidentiality

Confidentiality is not absolute and may be broken under specific circumstances, including:

  • If you are at risk of harming yourself or others.

  • If there is suspected abuse of children, elderly, or vulnerable adults.

  • If required by a court of law or subpoena.

6. Termination of Services

You have the right to terminate services at any time. We also have the right to terminate services if the therapeutic relationship is not working, if there are payment issues, or if other treatment needs arise that are outside the scope of our services.

7. Dispute Resolution

If a dispute arises, we will first attempt to resolve it through discussion. If the issue cannot be resolved through discussion, both parties agree to resolve the matter through mediation or arbitration.

8. Changes to Terms

We reserve the right to update or change these Terms at any time. You will be notified of any significant changes, and continued use of services after changes to the Terms constitutes acceptance of the new Terms.
